La misura mira a migliorare l'assistenza delle persone affette da patologie croniche, con particolare attenzione verso gli over 65.
A questo obiettivo primario si collega l’obiettivo complementare di aumentare il numero dei pazienti assistiti nelle proprie abitazioni.
Per la Regione del Veneto l’obiettivo riguarda la presa in carico del 10,98% della popolazione over 65 in ADI (Assistenza Domiciliare Integrata).

The measure aims to improve the care of people with chronic illnesses, with a focus on the over-65s. Linked to this primary objective is the complementary goal of increasing the number of patients cared for in their own homes. For the Veneto Region, the goal concerns taking care of 10.98% of the over-65 population in IHC (Integrated Home Care).

La misura mira a migliorare l'assistenza delle persone affette da patologie croniche, con particolare attenzione verso gli over 65.
A questo obiettivo primario si collega quello di realizzare un nuovo modello organizzativo, con la creazione delle Centrali operative territoriali, al fine di assicurare la continuità, l'accessibilità e l'integrazione della cura sanitaria,
nonché di coordinare i servizi domiciliari con gli altri servizi sanitari, assicurando l'interfaccia con gli ospedali e la rete di emergenza-urgenza. Il finanziamento in particolare riguarda l’acquisizione di sistemi informativi per l’interconnessione.

The measure aims to improve the care of people suffering from chronic diseases, with a focus on the over-65s.
Related to this primary objective is the implementation of a new organizational model, with the creation of Territorial Operations Centers, in order to ensure the continuity, accessibility and integration of health care, as well as to coordinate home services with other healthcare providers,
ensuring the interface with hospitals and the emergency- urgency network. Funding specifically covers the acquisition of information systems for interconnection.

L’obiettivo si traduce nell’acquisire device di categorie merceologiche diverse per rendere maggiormente efficace l’intervento del personale medico ed infermieristico delle COT, affinché possano rilevare i parametri vitali del paziente direttamente presso il domicilio e da qui,
in alcuni casi, trasmetterli attraverso rete di telemedicina o altra infrastruttura di rete e sistemi informativi adeguati, alla cartella clinica elettronica e al fascicolo sanitario elettronico del paziente (FSE).

The goal translates into acquiring devices of different commodity categories to make the intervention of medical and nursing staff of the Territorial Operations Centers more effective, so that they can detect the patient's vital parameters directly at the patient's home
and from there, in some cases, transmit them via telemedicine network or other appropriate network infrastructure and information systems, to the electronic medical record and the patient's electronic health record (EHR).
